Let’s consider the day

– Percy screws Izzie over with the research library code. So she’s miffed at him.

– Percy picked up the labs and offers to do the Dialysis, Izzie refuses and they get into a minor spat about personal vs professional

– She enters the room angry, does not look at the lab notes, and orders a Dialysis tech to start the machine unaware that the machine needs to be adjusted for her Potassium Levels.

– The Machine sucks all the potassium out of her body, and while she survives she cannot get her kidney. If a replacement is not found, Izzie will have killed her.

Percy was being an ass to Izzie, but she did not look at the lab notes due to being angry at him. Considering her patient was to receive an organ, it would have behooved her to review the labs as a standard dialysis was not going to be good for her. The Dialysis Tech might have noticed something, but as the doctor it’s all on Izzie the second she gave the order.

Sorry, I hate him too, but this whole issue with Sarah Freemont was Izzie’s fault.
